Insurance
We accept most major insurance plans. If you are unsure if our practice is an in-network provider with your specific plan, please call your insurance carrier ahead of your appointment to avoid unintended charges not covered by your insurance. If your insurance plan requires a referral, please request a referral from your primary care physician several days before your appointment to allow time for processing. Copayments vary based on insurance plans, and we collect a copayment as a specialist provider at the time of service. Copayments are collected for each visit.
Late Arrival/No Shows
Because of the high volume of patients in our office, anyone who is more than 15 minutes late will have to reschedule for another date. If you are unable to make your appointment or if you are going to be late, please call to make arrangements. Same day cancellations will be assessed a missed appointment fee of $25.

Medications
Please bring a complete list of medications and drug allergies to every appointment. Most patients have more than one physician who prescribes medications, and it is very important that we keep an updated list in your chart to avoid unnecessary side effects and drug interactions. If you are prescribed a new medication and are unsure if it is safe to take with a medication that we have prescribed, check with a pharmacist or call our office.
